You request your information statement
When you terminate the insurance contract for your vehicle and wish to insure the same vehicle or a replacement vehicle with another insurer, the latter will ask you to send them a copy of your information statement. This is a document containing the claims declared over the five years preceding the preparation of the document and mentioning your share of responsibility for each claim. This document also specifies the reduction-increase coefficient (bonus-malus) applied to the last annual deadline. It allows your new insurer to assess the premium you will have to pay to insure your vehicle.
Your insurer must send you this document when you terminate the contract, but you can also request it at any time. The latter has fifteen days to respond to you. If, after this period, you have not received a response, do not hesitate to start it again.
Dear,
(First letter) Thank you for sending me a statement of information within a fortnight for the contract n ° (reference of the insurance contract) covering the registered vehicle (vehicle registration number),
Pending your response, please accept, Madam, Sir, the expression of my best regards.

Dear,
(Reminder letter) On (date of your first letter), I asked you to send me an information statement for the registered vehicle (vehicle registration number) insured with your company under the n ° (reference of the insurance contract).
However, unless I am mistaken, this document has still not been sent to me.
I would like to remind you that, pursuant to article 12 of the appendix to article A. 121-1 of the Insurance Code, you are required to send me this document no later than fifteen days from counting from my express request.
I therefore hereby reiterate my request and count on your diligence.
Please accept, Madam, Sir, the expression of my best regards.
