Clinician Packet Template — Ready-to-Share Folder (Text Checklist)









Use this exact folder structure and file list when exporting sleep data to a clinician. Save everything in one zipped folder and label it clearly: SleepPacketYourName2026.zip

- Summary.csv — single CSV row per night with columns: Date, Bedtime(local), WakeTime(local), TotalSleepMinutes, SleepEfficiency%, DeepMin, REMMin, LightMin, Awakenings, SleepScore, HRAvg, HRVAvg, SnoreEventCount, SnoreIntensityAvg, Tags.  

- RawNightClips/ — 3–7 short audio clips (30–60s) labeled by date and time: 2026-03-0102-14snore.mp3.  

- DeviceExport/ — original device export files (CSV, JSON) if available.  

- WearableHRVCharts.pdf — 1–2 exported charts showing HR and HRV trend across monitoring period (png/pdf).  

- SymptomDiary.txt — daily notes: Sleepiness (ESS score), morning headaches, naps, medication, caffeine/alcohol, partner reports.  

- InterventionsLog.txt — conservative steps already tried (positional change, weight loss attempt, alcohol cutoff), with dates.

Export includes 18 nights, 5 flagged nights with high snore intensity and HR spikes. How to Export from Common App Types (Textual “Screenshots” / Steps)


Phone-only app (typical)

1. Open app > Settings or Profile.  

2. Data / Exports / Download. Choose CSV + Audio Clips.  

3. Select date range (e.g., 2026-08-01 → 2026-08-21). Export to device storage.  

4. File manager: move exports to folder SleepPacketYourName2026.


Wearable app (typical)

1. Open wearable app > Profile > Data export.  

2. Export HR and HRV ranges as CSV and charts as PNG.  

3. Save to device or sync to desktop via companion app. Move files to DeviceExport/ in packet.


Under-mattress sensor (typical)

1. App > Account > Data > Export. Choose nights and export.  

2. Include event logs (movement, breathing) and summary CSV. Save to DeviceExport/.


Smart-home logs (optional)

- If you used light-automation schedules, include a short text file: SmartHomeLog.txt listing scenes triggered and relevant timestamps (helps clinicians correlate light therapy with sleep changes).


Tip: If an app auto-uploads to cloud only, use the app’s “request data” or “download my data” feature and wait for the export link (typically emailed). Download immediately and move to your packet.

ESS (Epworth Sleepiness Scale) — Quick Self-Assessment (Copy-Paste)


Instructions: Rate the chance of dozing (0–3) in each situation. Total score = sum (0–24). Higher scores mean greater sleepiness.


1. Sitting and reading: 0 / 1 / 2 / 3

3. Sitting inactive in a public place (e.g., theater): 0 / 1 / 2 / 3

Interpretation notes (for clinician reference): 0–7 normal; 8–9 mild sleepiness; 10–15 moderate sleepiness; 16–24 severe sleepiness.


Add your total and include it in SymptomDiary.txt.

Quick Checklist Before You Send Anything to a Clinician


- Remove unrelated personal audio. Keep clips short (30–60s).  

- Ensure filenames include dates and times.  

- Confirm CSV headers are human-readable. Rename column headers if needed (Date, SleepScore, HRAvg, HRVAvg, SnoreCount, Tags).  

- Zip the folder and password-protect if emailing (share password via SMS).
